Kindly check the screen shot of workflow log it is in process state and the task "Leave request form is ready for the action to be performed by the agent.

agents symbol and check whether your user id is assigned there are not in possible agents.
This can be done in SBWP->Settings->Workflow Settings->Refresh Org environment.
or also It could be an authorization problem. Check if the user has the role SAP_BC_SRV_USER (Authorizations for the end users of the Business Workplace)
